| 13 | Incomplete or incorrect telephone or Toll-Free Number on form |
| 14 | Number in Reserve, Spare or Transitional Status |
| 15 | Not allowed per authorization of Submitter |
| 16 | Already completed or duplicate form |
| 17 | Required or accompanying paperwork missing |
| 18 | Required fields on form not completed |
| 19 | Not a current form |
| 20 | Not a valid or incorrect Resp Org listed |
