Toll-Free Subscriber Last Name	Last name of the Toll-Free Subscriber.	Required field. Maximum Length, 200 characters.
Subscriber Address 1	The Address line 1 for the Toll-Free Subscriber.	Required field. Maximum Length, 500 characters.
Subscriber Address 2	The Address line 2 for the Toll-Free Subscriber.	Optional field. Maximum Length, 500 characters.
Subscriber City	The City in which the Toll-Free Subscriber lives/works/resides.	Required field. Maximum Length, 100 characters.
Subscriber Zip Code	The Zip Code in which the Toll-Free Subscriber lives/works/resides.	Required field. Maximum Length, 50 characters.
Subscriber State	The State / Province in which the Toll-Free Subscriber lives/works/resides.	Required field. Must be a Valid State/Province Code.
Authorized Contact Name	Name of the authorized contact for the Toll-Free Subscriber.	Required field. Maximum Length, 200 characters.
Contact Title	Title of the Toll-Free Subscriber.	Required field. Maximum Length, 200 characters.
Contact Telephone #	Telephone number of the Toll-Free Subscriber.	Required field. Maximum Length, 50 characters.
Ext #	Telephone number's extension of the Toll-Free Subscriber.	Optional field. Allowable Values are Numeric with Optional "-".
Email	Email address of the Toll-Free Subscriber.	Optional field. Special characters allowed are !#$%&'*+-/=?^_`.{\|}~. Maximum length of 60 characters. Email address must be in the format: A@B.C (e.g., jdoe@myworkemail.com).
Resp Org Instructions	Notes from the Responsible Organization (Resp Org).	Optional field. Maximum Length 300 Characters. Allow all alphanumeric and special characters except &, ", ‘, < and >.
End User Information	Notes from the Toll-Free Subscriber.	Optional field. Maximum Length 700 Characters. Allow all alphanumeric and special characters except &, ", ‘, < and >.
