Start NXX |
Text field to begin a search for a TFNs at a specific NXX. Example: If the Start NXX is 231, the TFNRegistry™ will begin its search with 231-0000. The search will continue through higher numbers until the requested quantity of numbers is found or until the highest NXX is reached. |
3-digit number within the range of 000-999. Only digits (numeric) are supported. |
Start Line |
Text field to begin the search at a specific line number. Example: If the Start Line is 3456, the TFNRegistry will search only for numbers that end with the lines 3456 or higher line number, regardless of the NPA- NXX, such as searching NPA-NXX-3456 through NPA-NXX-9999. The search continues until the requested quantity of numbers is found or until all the applicable NPA-NXX codes have been searched. |

Contact Name |

Template |
A mandatory field that identifies the name of the Template Record (TAD) (also known as a TR) that is associated to the Pointer Record (PAD) (also known as a PR). A TR contains the routing for the PR.

Template Name must start with an asterisk * followed by 2 letters (i.e., the Entity code) and then followed by 1-12 alphanumeric characters. Alphanumeric characters are digits (numbers 0 through 9) and/or letters (A through Z). Minimum total data width is 4 characters and maximum total data width is 15 characters when counting the leading asterisk. Dashes are optional and allowed only in character positions 4-15. |
Number of Lines |
Required field. The number of lines on which the TFNs may terminate per each Number (for the Number entered on same line). |
1 to 4 numeric. Ex: 1 to 9999 |
Service Order |
Required field. The Service Order type and number of Service Order must be provided. |
4 to 13 alphanumeric characters. The first character must be alphabetic. The 2nd to 12th characters can be alphanumeric. If there is a 13th character it must be alphabetic. |
Time Zone |
The Time Zone is auto populated from the URC page and the user's preferred Time Zone setting. |

Effective Date |
Required. The Effective Date identifies the date when the record is activated. This field can be entered manually or by using the Calendar pop-up. |
MM/DD/YYYY Format. When “Now” is checked, this field will be disabled.

Effective Time |
Optional. The Effective Time identifies the time when the record is activated. This field can be entered manually. |
HH:MM AM/PM. When “Now” is checked, this field will be disabled. |
Now |
Optional. Current date and time will be used during creation of the request when "Now" is selected. |
